We are seeking a Full-Time Registered Practical Nurse (RPN) to support patient care delivery and the implementation of programs that will optimize the health of our patients. Reporting to the Human Resources and Operations Manager, this position will work as part of the team providing direct primary care for patients.
Key Responsibilities:
" Administer medications and treatments and accurately monitor and document vital signs and symptoms.
" Provide patient and family health education using effective, needs-based teaching strategies
" Organize and deliver patient care in collaboration with patients, families, and the interprofessional team
" Coordinate diagnostic tests and procedures with physicians or according to medical directives
" Maintain clear, accurate EMR documentation to support communication across the care team
" Establish respectful, collaborative relationships within the Family Health Team and the circle of car
" Protect confidential and sensitive patient information and uphold professional standards
" Support practice improvements that enhance nursing care and patient outcomes
" Perform other related duties as assigned by the Human Resources and Operations Manager
As a member of the Port Arthur Health Centre Family Health Team, you will:
o Collaborate with community and hospital partners to enhance programs and patient care services.
o Participate in the development, implementation, and improvement of patient programs.
o Contribute to the overall functioning and success of the Family Health Team.
o Engage in quality improvement activities, including data collection and evaluation.
o Support a healthy, collaborative workplace that values learning, diversity, privacy, and professional standards.
Qualifications:
" RPN license from College of Nurses of Ontario in good standing
" Recent related nursing experience; prefer two years' working experience in a community-based or hospital-based clinical team
" Proof of current BCLS/CPR/First Aid Training
" Proof of Professional Liability Insurance
" Experience working as part of an interdisciplinary team
" Comfortable with electronic documentation and use of technology in the workplace
