Position:   Nurse Practitioner

We are seeking two Full-Time permanent Nurse Practitioner to support patient care delivery and the implementation of programs that will optimize the care of our patients. As a registered and licensed Nurse Practitioner, you provide patient centered care under the direction of physicians, focusing on health promotion, disease prevention and treatment. Reporting to the Human Resources and Operations Manager, the Nurse Practitioner will work as part of the team to deliver care and help develop programs such as diabetic management, cervical cancer screening, well baby checkups and more. Working to the full scope of their practice, the Nurse Practitioner will also work independently to assess, diagnosis and treat patients seen in the practice.

Responsibilities:

  • Provide acute, episodic, and chronic patient care, including comprehensive assessments, diagnosis, treatment planning, and EMR documentation.
  • Monitor patient progress, administer medications and treatments, and assist with diagnostic tests and minor procedures.
  • Educate patients on health management, medications, and preventive care.
  • Collaborating with physicians and circle of care to coordinate and optimize patient care
  • Develop and support patient programs and contribute to team education resources and workshops
  • Arrange specialist referrals and communicate assessment findings within the circle of care.
  • Ensure compliance with infection control, safety standards, and confidentiality requirements.
  • Participate in quality improvement initiatives, team meetings, and the development of clinical programs.
  • Maintain professional relationships with community networks and provide guidance to team members within scope.
  • Perform other duties as assigned by the Human Resources and Operations Manager.

  • Participate in the development, implementation, and improvement of patient programs.
  • Contribute to the overall functioning and success of the Family Health Team.
  • Engage in quality improvement activities, including data collection and evaluation.

Qualifications:

  • RN license from College of Nurses of Ontario in good standing, designation required
  • Completion of Nurse Practitioner (NP) Primary Care certificate is required
  • Master's degree in nursing is required
  • Recent related nursing experience: Prefer five years' working experience in a community-based or hospital based clinical team delivering related programs
  • Proof of professional liability insurance
  • Proof of BCLS Certification
  • Experience working as part of an interdisciplinary team
  • Comfortable with electronic documentation and use of technology in the workplace
